- Coordinate and process referrals
- Coordinate and process hospital discharges
- Submit and track authorizations
- Interface with corporate management, administrative personnel, medical, dental and behavioral staff
- Identifies the health needs of the patient and the expectations of the family.
- Makes an assessment of the resources and support network at the patient’s disposal.
- Assists in development, implementation, and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan; and monitors progress toward treatment goals.
- Maintains an active caseload, providing interventions as needed and within area of expertise and limits of credentials; assures procurement of additional services as needed.
- Reviews and facilitates, if appropriate, mental health referrals from other sources such as primary care, school health, etc. Offers alternative recommendations where necessary.
- Maintains patient and program records in accordance with applicable standards and regulations, grant requirements, etc.
- Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
- Participates in continuing education activities, remaining knowledgeable in area(s) of expertise.
- Attend meetings, as appropriate, and meet regularly with supervisor to exchange pertinent information and receive supervision.
- Participates in all safety programs, which may include assignment to an emergency response team.
- Other responsibilities as assigned to create a smoothly run efficient Clinic.
- High School Diploma or equivalent (required).
- Associate's degree in health administration, Medical Office Administration, ora related field (preferred).
- Certification in medical office administration ora related certification (e.g., Certified Medical Assistant, Referral Coordinator certification) is a plus.
- Minimum of 1-2 years of experience in a healthcare setting, preferably in a referral coordination, medical office, or patient services role.
- Experience with insurance verification and authorization processes is highly desirable.
- Familiarity with electronic health record (EHR) systems and scheduling software.
- Knowledge of medical terminology, patient referral processes, and healthcare regulations, including HIPAA compliance.
- Experience working in a Federally Qualified Health Center (FQHC) or community health setting is a plus.

Skills Required
- High School Diploma or equivalent
- Associate's degree in Health Administration or related field
- Certification in medical office administration or related certification (e.g., CMA, Referral Coordinator certification)
