Description

Under the supervision of the Supportive Housing Manager, the Case Manager provides case management and crisis intervention services to residents of the Recovery House program using a recovery-oriented, trauma-informed, and person-centered approach. This role supports formerly homeless individuals in maintaining stable housing, strengthening recovery, and achieving greater independence while fostering a positive and supportive residential community.

Using a strengths-based, harm-reduction approach, the Case Manager helps residents navigate challenges, build resilience, and work toward individualized goals. The position collaborates with residents, service providers, and community partners to coordinate access to mental health, substance use, medical, vocational, and other supportive services that promote housing stability, recovery, and successful community integration.

Requirements

  • Bachelor’s degree in the field of Social Work/Human Services preferred.
  • Minimum of two years of professional experience working directly with clients who have a variety of behavioral health needs, specifically history of mental health and substance use disorders.
  • Familiarity with the physiological, psychological, social, and economic effects of homelessness.
  • Must demonstrate appreciation and willingness to develop continuing skills towards multicultural competency within a behavioral health setting.
  • Bilingual (English-Spanish) preferred.
  • Must have an appreciation of culturally relevant services and demonstrate willingness for the provision of the commitment to such services.
  • Must be a growth-oriented person willing to set and work toward professional goals.
  • Must have a valid driver’s license and motor vehicle.

Skills Required
