Job Title: Quality Records Auditor- Referrals

Position Summary
Conducts audits of referral records for accuracy of documentation. Conducts audits of hospital death record
reports for accuracy and completion. Acts as a quality resource for the Hospital Development team.
Essential Functions
- Must be able to effectively operate Google suite applications as well as learn and adopt computer skills to effectively update and communicate within programs such as iTx, Hospital Electronic Medical Records
- Systems (EMRs), and LOPA’s Electronic Quality Management System (eQMS).
- Must be able to effectively and professionally communicate via different medias including, but not limited to, verbal, written and digital medias. Communication may be both for internal and external audiences.
- Must be able to utilize current organizational resources for procedural compliance to determine next steps in the process.
- Must be able to applying reasonable clinical judgment while adhering to established algorithms to guide and determine appropriate courses of action.
Job Role Expectations
- Communicates and collaborates with designated points of contact at hospitals to obtain monthly hospital death lists and required records to conduct reviews of deaths.
- Performs a systematic review of hospital patient death records to evaluate care processes, identify opportunities for improvement, and ensure compliance with applicable CMS Conditions of Participation, including timely referral to LOPA when required.
- Demonstrates flexibility and adaptability in implementing process changes and adapt to ensure timely and accurate management of incoming referrals.
- Assess hospital medical records to confirm missed organ or tissue referrals.
- Accurately documents findings of review in LOPA’s electronic donor record.
- Audits electronic records of completed ventilator referrals to ensure completion of critical data points for internal and external reporting purposes.
- Accurately documents audit findings for referrals in LOPA’s electronic medical record.
- Completes record reviews, requests, and reports within the specific timeframe.
- Maintains access to hospital electronic medical record systems.
- Proactively identifies and reports improvement opportunities.
- Participates in Hospital Development staff meetings, Family Services Meetings, required educational activities, and identified quality improvement opportunities.
- Completes assigned eQMS and Learning Management System tasks by the assigned due date.

Work Environment
- Possible exposure to communicable diseases, bloodborne pathogens, airborne illnesses, hazardous materials, pharmacological agents with little likelihood of harm if established health precautions are followed.
- May, at times, have exposure to blood, packaged organs and tissues for transplant and/or research in a hospital setting or while in the office.
- Work is done indoors in an office setting.
Physical Demands
- Sedentary work that primarily involves sitting, some opportunities for standing work and breaks.
Work Hours
- Monday through Friday 8:00 am – 4:30 pm.
- This position follows a hybrid work arrangement. Employees may be eligible to work remotely 1–2 days per week, subject to business needs, performance, and manager approval.
Requirements
Education and Experience
- Graduate of nursing school or other medical degree or 6 months experience in a clinical environment or a combination of education and experience

Skills Required
- Graduate of nursing school or other medical degree OR 6 months experience in a clinical environment (or combination)
- Working knowledge of Google suite applications (Google Workspace)
- Ability to learn and use iTx, hospital Electronic Medical Records (EMRs), and LOPA's eQMS
- Ability to effectively communicate verbally, in writing, and digitally with internal and external audiences

As a non-profit, LOPA is dedicated to saving lives, restoring health, and enhancing medical care through organ and tissue donation. The organization supports families through the donation process and their grief journey, while educating professionals and the community about the life-saving impact of donation.
