JOB PURPOSE: 

 

The Program Coordinator will work within the Endeavors Transition Support Program (TSP) with the primary responsibility of providing case management support to transitioning service members enrolled in Onward Ops.  

 

The Program Coordinator will establish and maintain supportive relationships with participants, assess individual needs, identify and locate appropriate community and VA resources, and facilitate connections to services that support successful transitions. Responsibilities will include conducting Columbia-Suicide Severity Rating Scale (C-SSRS) screenings, developing safety plans when indicated, providing ongoing follow-up and support, documenting participant progress, and coordinating referrals to behavioral health, wellness, and other community resources.  

 

The Program Coordinator will monitor participant engagement, identify barriers to accessing services, and collaborate with program partners to ensure participants receive timely and appropriate support. 

Qualifications

ESSENTIAL JOB RESPONSIBILITIES: 

 

  • Serve as a primary point of contact for transitioning service members enrolled in the Transition Support Program and provide timely, supportive communication throughout their participation. 

  • Conduct initial outreach and engagement with transitioning service members 

  • Provide case management services by assessing participant needs, identifying barriers to successful transition, locating appropriate community, VA, behavioral health, employment, housing, financial, and other resources, and facilitating connections to services. 

  • Conduct Columbia-Suicide Severity Rating Scale (C-SSRS) screenings in accordance with program protocols and respond appropriately to identified levels of suicide risk. 

  • Develop and maintain individualized safety plans with participants when indicated and provide appropriate follow-up to support participant safety and engagement. 

  • Provide ongoing follow-up with transitioning service members to monitor progress, address emerging needs, support engagement with referrals, and identify additional resources or services as needed. 

  • Document participant contacts, assessments, referrals, services, and outcomes accurately and in accordance with program requirements. 

  • Monitor participant engagement and caseload activity to identify individuals requiring additional outreach, support, or intervention. 

  • Collaborate with multidisciplinary team members and community partners to coordinate services, facilitate warm handoffs, and support continuity of care and resources for transitioning service members. 

  • Support the development and maintenance of relationships with community and VA resource providers to expand available resources and referral options for participants. 

  • Provide interim support to transitioning service members who are awaiting additional services or who have identified needs requiring increased contact or assistance. 

  • Work with other program support staff to increase service delivery and enhance participant outreach and engagement. 

  • Prepare and maintain client data to support leadership updates, program monitoring, evaluation, and continuous improvement. 

  • Collaborate with the multidisciplinary team as needed to support program initiatives, special projects, community outreach, and implementation activities. 

  • Other duties as assigned. 

 

 

 

ESSENTIAL QUALIFICATIONS:   

   

EDUCATION: Bachelor’s degree preferred 

 

EXPERIENCE: Experience working with Veterans, Service Members, and their families.   

 

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion. Employee is expected to report to work as scheduled, on time, and prepared to start work. This is a remote role requiring the employee to have the ability to remain on task in a work from home environment. 

 

 

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
