Job Description:

Schedule Information:

Monday through Thursday, in person.

Flexible hours: 8:00 a.m.–4:00 p.m. or 9:00 a.m.–5:00 p.m.

Fridays are flexible and coordinated in advance
Participates in departmental and institutional committees, representing pharmacy services and contributing to performance improvement projects. Coordinates Clinical Pharmacy Program Development and Quality Initiatives- Designs and implements pharmacy programs, quality improvement efforts, and medication stewardship initiatives to enhance medication-use systems and patient outcomes. Facilitates Formulary Management and Clinical Services- Develops treatment guidelines, formulary recommendations and protocols. Supports P&T Committee preparation and provides consultation on therapeutic appropriateness. Directs Quality Assurance and Performance Improvement- Develops and monitors quality metrics, leads improvement initiatives and benchmarks performance against internal and external standards. Coordinates Pharmacy Program Development and Quality Initiatives- Leads the design, implementation, and continuous improvement of pharmacy programs, including clinical services, stewardship, education, utilization reviews and operational workflows. Promotes optimal patient outcomes and medication-use safety. Facilitates Clinical Services and Formulary Management- Develops treatment guidelines, formulary recommendations, and medication-use protocols. Prepares materials for P&T Committee and provides consultation on therapeutic appropriateness and utilization trends. Collaborates Across Departments And Committees- Represents pharmacy in interdisciplinary committees, task forces, external organizations to advance institutional goals. Participates in Medication Safety And Error Reduction Efforts- Identifies opportunities for system improvements based on medication event data. Supports variance reviews contributes to safety committee activities. Coordinates Education And Staff Development- Facilitates training for new staff, supports ongoing education, monitors competency completion. Serves as a preceptor for students and residents. Monitors Clinical, Operational and Financial Performance- Tracks KPI's, budget adherence and resource utilization. Benchmarks performance against internal targets and national standards to guide strategic decision.
Supports Pharmacy Informatics and Technology Optimization- Guides EHR and automation system enhancement, projects management and workflow integration. Maintains expertise in pharmacy informatics and data analytics to support operational efficiency. Engages in Professional Development- Serves as a preceptor, participates in professional organizations and contributes to pharmacy practice advancement. Other duties as assigned.

Education:

  • Bachelor's [Required]

Field of Study:

  • in Pharmacy

Work Experience:

  • 3+ years of current hospital pharmacy practice experience [Required]

  • Practice experience in a large hospital (greater than 500 bed institution) and integrated healthcare system (three or more hospitals in system. [Preferred]

Licenses and Certifications:

  • Licensed Pharmacist (RPH) [Required]

  • Basic Life Support - CPR Cert (BLS) [Preferred]
