Position Summary

The Pharmacist will be responsible for developing, implementing, and maintaining initiatives that ensure medications prescribed for kidney and cardiac disease, and their related comorbidities are evidence-based and cost-effective. The role involves addressing pharmacy referrals and assisting with medication access, education, and adherence to support kidney and cardiac health management. The Pharmacist will work collaboratively with clinical and care management teams to identify medication opportunities and resolve medication safety concerns associated with transitions of care and chronic disease management. The Pharmacist will guide Healthmap’s comprehensive pharmacy strategy and work collaboratively across Healthmap departments to bring value to all pharmacy related initiatives and clinical programs.

Responsibilities 

  • Review medication regimens and make recommendations on how regimens might be adjusted based on protocols and current guidelines to optimize disease management
  • Create educational material for providers, members, and staff regarding new drugs, new indications, guideline updates, and/or counter-detailing of industry advertisement
  • Participate in the development of treatment algorithms, guidelines, policies and procedures as well as other kidney and cardiac disease management efforts
  • Create written responses to pharmacy referrals related to medication access, affordability, education, adherence, optimization, and safety
  • Evaluate medications that are high-cost, high-risk or high-volume using population health framework
  • Enhance processes related to medication reconciliation and medication adherence
  • Collaborate with other care providers in the effective sharing of practice expertise
  • Oversee pharmacy technicians
  • Participate in quality improvement activities and process enhancement initiatives as assigned
  • Adhere to policies, procedures and state and federal regulations to ensure compliance and confidentiality of Protected Health Information (PHI). Participation in Compliance and other important training is a condition of employment
  • Perform other duties as assigned

Requirements

  • Active Pharmacist License 
  • PharmD degree from an accredited School of Pharmacy 
  • 5 years clinical pharmacy experience in working face to face with physicians and health care providers in hospital or ambulatory care
  • Renal patient pharmacy management and value-based care experience, Health Plan and/or Vendor experience preferred
  • American Society of Health Systems Pharmacists (ASHP) Accredited Residency preferred
  • Understanding of Centers for Medicare and Medicaid Services (CMS) Star measures, Healthcare Effectiveness Data and Information Set (HEDIS) measures, MIPS and National Quality Forum measures

- Occasional movement of equipment, or materials up to 50/75 Pounds

- Occasional bending, twisting, and stooping
- Occasional need to kneel or crouch
 
Additionally Traveling positions (ex. Field Care Navigators or Quality Practice Advisors) may require:
• Sense Acuity
- Ability to see/read road signs, maps, and electronic devices
- Good visual acuity for driving, including peripheral vision and depth perception
• Environmental
- Possible exposure to travel, traffic and/or other outdoor hazards
- Possible exposure to various weather conditions, including extreme heat, cold, rain, and snow

Skills Required

  • Active pharmacist license
  • PharmD degree from an accredited school of pharmacy
  • Five years of clinical pharmacy experience working face-to-face with physicians and healthcare providers in hospital or ambulatory care settings
  • Renal patient pharmacy management and value-based care experience
  • Health plan and/or vendor experience
  • ASHP-accredited residency
  • Understanding of CMS Star measures, HEDIS measures, MIPS, and National Quality Forum measures
