Job Description

Project management and performance improvement support for teams within the CICIP and Population Health portfolio.

Responsibilities

  • Coordinate projects within the Care Innovation and Community Improvement Program (CICIP) - this position will assist with half of the CICIP portfolio, which covers Maternal & Infant Health, Access to Mental Health, Addressing Opioid Prescribing & Pain Management, Improving Access to Care (Reducing ED Utilization), and Addressing Substance Use Disorders, in total.
  • Connect the projects and teams within designated portfolio to partners at other CICIP hospital systems within the statewide coalition to share our best practices and learn from other healthcare system successes.
  • Facilitate utilization of the UCH PI Way by means of assisting CICIP teams with the performance improvement process.
  • Drive change by working with clinical providers, staff and content experts to identify and implement evidence based care.
  • Manage metrics necessary to track project performance and processes in partnership with system data analysts.
  • Lead patient engagement opportunities with working teams to ensure that the voice of the patient drives forward project direction.
  • Work independently to develop, implement and manage projects; knowledge of the principles and concepts of project management including proficiency with relevant project management tools.
  • Communicate and execute standard roles and responsibilities for project team members.
  • Facilitate continuous cadence and structure of leadership and project team meetings with an emphasis on driving forward improvement toward program quality goals, as well as making problems, barriers and results clear with minimum of administrative burden.
  • Drive forward innovative approaches to the greatest health needs identified by project teams and support operationalization of future project planning.
  • Understand and meet expectations for deliverable quality and timeliness for project plan progression.
  • Connect work within the CICIP portfolio to larger initiatives within the Population Health department and act as liaison with additional stakeholder groups across the health system.
  • Support the system culture of continuous improvement through promotion of the UC Health PI Way.

Ability to work independently to develop, implement and manage projects; knowledge of the principles and concepts of project management including proficiency with relevant project management tools Drive change by working with faculty and content experts to identify evidence based care. Work with leaders and management to plan and lead planning group meetings. Orient group to the process and their roles and responsibilities. Coordinate learning opportunities with guidance from management and faculty. Develop agendas. Support recruitment of participants by interacting with staff and interested teams Coordinate and contribute to the development of learning objectives for all activities and develop evaluation plans. Create work products and project deliverables under direction of more senior-level team members. Understand and meet expectations for deliverable quality and timeliness. Provide planning and facilitation for meetings including large group learning sessions, conference calls, and expert meetings.

Work as a coordinator to encourage the team(s) take on the leadership of improvement work. Be a resource for how to use performance improvement tools Support the UC Health way the overall performance improvement system of how we get work done. Support routine (standard work) meetings, reviews and reports with an emphasis on making problems, barriers and results clear with a minimum of administrative burden.

Qualifications

Minimum Required: Bachelor's Degree in Clinical, Engineering, Business, Public Health or a related field

Formal performance improvement training desirable; Formal Data Management training desirable

Minimum Required: 2-3 years; experience in a related job discipline; Preferred: 1-3 years equivalent experience in healthcare
