The Role

The Nurse is a key member of the Circulo Health team who cares for the health of our members as a way to ensure the success of the Health Hub.

What We Expect from You:

• Ready to manage a panel of members (from infants to older adults) to support the member’s health goals holistically
• Experienced in comprehensive care management, chronic disease management, urgent home-based and community-based primary health visits, preventative care, and wellness.
• Experienced with virtual member engagement, member-centered action and/or care plans, resource management.
• Comfortable visiting members in their homes. Ability to travel within Clifton Park/Albany, NY area to complete home visits.
• Strong communication and relationship skills with demonstrated success partnering with providers and other team members to support wellness and prevention.
• Adaptable, organized, efficient, independent self-starter and problem-solver, a leader, a strategic thinker, and a mentor, who is excited about the big picture of whole community health.
• Willing to commute 2-3 days to Clifton Park, NY location

Degree Requirements and Certifications:

• Registered Nurse with active licensure in the State of New York
• Minimum 2 years direct patient experience
• CPR certification required
• Outpatient/ primary care experience preferred
• Pediatric experience preferred
• Experience with Intellectual and Developmental disabilities is a plus
