Job Summary:

Performs a variety of duties to provide administrative support to the Case Management Department.

Job Responsibilities:

  • Assists the Case Manager with facilitating discharges for patients on nursing units and identifies barriers to discharge.

  • Coordinates post-acute care referrals and transfers and documents in the electronic medical record.

  • Communicates with post-acute care facilities and other community agencies by phone and electronic medical record.

  • Coordinates and schedules transportation and appointments.

  • Completes required forms, regulatory letters, and other written communications and scans to electronic medical record.

  • Maintains assigned calendar(s).

  • Establishes and maintains filing systems.

  • Orders and stocks supplies.

  • Updates community resource information.

Skills and Attributes:

  • Requires critical thinking skills and decisive judgment.

  • Works under minimal supervision.

  • Must be able to work in a stressful environment and take appropriate action.

  • Must be able to multi-task
