Chartiers Center is a private, non-profit behavioral health care organization serving Southwestern Pennsylvania. As a Mobile Case Manager with Chartiers Center, you will have the opportunity to make a meaningful impact in the lives of adults living with serious mental illness and co-occurring conditions, including substance use disorders, and physical health challenges. In this dynamic, community-based role, you will build supportive relationships, connect individuals with essential resources, and serve as an advocate for their well-being and success.

Job Duties Include:
- Meet with individuals in their homes and throughout the community to provide support, guidance, and service coordination.
- Conduct comprehensive assessments to identify strengths, needs, preferences, and recovery goals.
- Develop, implement, and monitor individualized service plans that promote stability, growth, and independence.
- Connect individuals with critical resources, including housing, healthcare, behavioral health services, benefits, employment opportunities, and community supports.
- Collaborate with healthcare providers, therapists, family members, and community agencies to ensure coordinated and effective care.
- Maintain regular contact with individuals to monitor progress, address challenges, and identify emerging needs.
- Serve as a strong advocate, helping individuals navigate systems and access the services they need to succeed.
- Complete accurate and timely documentation in accordance with agency and regulatory requirements.
- Participate in a shared on-call rotation, typically no more than once per month.

Minimum Qualifications:
- Associate’s degree in human services and 3+ years of mental health experience.
- Bachelor's degree in human services and 2 years of mental health experience OR
- Master’s degree in human services with one year of experience
- Valid Driver’s License Required
- Ability to travel throughout the community
