Intensive Mobile Case Manager

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program.  We take pride in being innovative in helping those we serve achieve personal goals by creating opportunities to align our services and supports with those goals and to help them in their recovery. 

The Janus Program is a new model of care that uniquely assists participants to liven on their own and learn desired skills.  The Intensive Mobile Case Manager is a member of the multi-disciplinary team that works closely with program participants. Although we embrace a "whatever it takes" approach the Enhanced case manager provides personalized and specialized Service Coordination to participants of the Janus Program.    

Job duties include:

• Coordinate and manage the delivery of behavioral health, medical social service and community support services.

• Assess participant needs and develop individualized, person-centered service plans.

• Meet with participants on a frequent basis to provide ongoing case management interventions and monitor participant progress. 

• Maintain accurate and timely documentation in compliance with organizational and regulatory requirements.

• Participate in rotating on-call responsibilities to provide after hours support for Janus program participants.

Minimum Qualifications:  

• Master’s degree in human services or related fields with 2 years of mental health experience   

• Bachelor’s degree in human services or related fields and 3 years of mental health experience OR   

• Associate's degree in human services or related fields and 4 years mental health experience

• Valid Driver’s License Required
