Greenville Nursing & Rehabilitation is seeking an experienced and detail-oriented Registered Nurse (RN) to join our leadership team as our MDS Coordinator. This key leadership position is responsible for overseeing the MDS process, supporting reimbursement accuracy, ensuring regulatory compliance, and collaborating with the interdisciplinary team to provide exceptional resident-centered care.

Schedule
  • Full-Time

  • Monday–Friday

  • Participation in the management on-call rotation is required

  • Additional hours as needed to support resident care and regulatory requirements

Compensation
  • $39.00 per hour

  • $10,000 Sign-On Bonus

  • Sign-on bonus paid bi-weekly over one year

Responsibilities
  • Coordinate and oversee all MDS assessments and submissions

  • Ensure compliance with federal, state, and facility regulations

  • Monitor assessment schedules and submission deadlines

  • Lead interdisciplinary care plan meetings and collaborate with department leaders

  • Develop and update individualized resident care plans

  • Monitor reimbursement opportunities and quality measures

  • Review clinical documentation for accuracy and completeness

  • Support survey readiness and regulatory compliance initiatives

  • Monitor changes in resident condition and ensure timely assessments

  • Serve as a clinical resource for staff regarding MDS processes and documentation

Qualifications
  • Current Illinois Registered Nurse (RN) license required

  • Previous MDS experience strongly preferred

  • Long-term care experience preferred

  • Strong knowledge of Medicare, Medicaid, reimbursement systems, and MDS processes

  • Excellent communication, organizational, and leadership skills

  • Ability to work independently while collaborating effectively with an interdisciplinary team

  • Experience with EMR and clinical documentation systems preferred
