Carrier Mills Nursing & Rehabilitation Center is seeking an experienced and motivated MDS Nurse Manager (RN or LPN) to join our leadership team. This key leadership position is responsible for coordinating the MDS process, supporting quality resident outcomes, maximizing reimbursement accuracy, and ensuring compliance with federal and state regulations.

Participation in the management on-call rotation is required

Registered Nurse (RN): $33.00–$35.00 per hour (based on experience)

Coordinate and oversee the MDS assessment process
-
Complete and submit accurate, timely MDS assessments in accordance with CMS guidelines
-
Lead interdisciplinary care plan meetings and collaborate with department leaders
-
Monitor case mix and reimbursement opportunities
-
Maintain compliance with federal and state regulations
-
Support quality measures, survey readiness, and regulatory compliance initiatives
-
Collaborate with nursing leadership to promote outstanding resident outcomes and accurate clinical documentation
-
Current Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required
-
Previous MDS and care planning experience required
-
Strong knowledge of Medicare, Medicaid, and reimbursement systems preferred
-
Excellent communication, organizational, and leadership skills
-
Strong attention to detail with the ability to prioritize multiple responsibilities

Long-term care experience preferred
