Job SummaryGreat resident care starts with great assessments—and Kimes Nursing and Rehab is looking for an experienced MDS Nurse (RN/LVN/LPN) who believes people always come first to join our team. This role is responsible for coordinating resident assessments, ensuring accurate documentation, and supporting quality care planning while maintaining regulatory compliance. Responsibilities

  • Coordinate and complete MDS assessments in accordance with CMS requirements and facility policy
  • Ensure timely and accurate MDS submission
  • Collaborate with the interdisciplinary team to develop and update care plans; collect and validate supporting documentation from Nursing, Rehab, Social Services, Dietary, and Activities
  • Monitor documentation to support PDPM and quality measures
  • Participate in care plan meetings with residents and families
  • Assist with survey readiness and audits
  • Educate clinical staff on charting best practices that support accurate MDS coding and QMs.
  • Review Quality Measures (QMs), 5‑Star metrics, and facility dashboards; help drive performance improvement initiatives.

Requirements

  • Active nursing license in good standing
  • MDS experience in skilled nursing or long-term care preferred
  • Working knowledge of PDPM, QMs, Care Area Assessments, and CMS submission processes.
  • Strong organizational and documentation skills
  • Passion for delivering excellent resident care
