Reports To: Administrator or Director of MDS

 

Chase City Health & Rehab Center is seeking an experienced, detail-oriented, and compassionate MDS Coordinator RN to join our clinical leadership team. The MDS Coordinator RN provides leadership and oversight for the Resident Assessment Instrument (RAI) process, care planning, quality measures, clinical reimbursement, and regulatory compliance.

 

RESPONSIBILITIES

• Coordinate and oversee the completion of MDS assessments, Care Area Assessments (CAAs), and comprehensive care plans

• Collaborate with nursing, therapy, social services, dietary, physicians, residents, and families to gather assessment information

• Ensure MDS assessments, care plans, and documentation are completed accurately and within required federal and state timeframes

• Maintain assessment schedules for all residents and communicate deadlines to interdisciplinary team members

• Review completed MDS assessments for accuracy, completeness, and regulatory compliance

• Ensure supporting clinical documentation is present to substantiate coding and reimbursement requirements

• Monitor care plans to ensure they accurately reflect residents needs

• Provide education and training to interdisciplinary team members regarding the MDS process and documentation requirements

• Support Quality Assurance and Performance Improvement initiatives

• Review and monitor case mix index data to ensure accurate representation of resident care needs

• Participate in interdisciplinary meetings and care planning discussions

• Lead clinical reimbursement discussions related to Medicare Part A, Managed Care, and other reimbursement programs

• Communicate effectively with residents, families, physicians, and healthcare team members

• Perform other duties as assigned

Qualifications

QUALIFICATIONS

• Current Virginia Registered Nurse (RN) license in good standing required

• Minimum of two years of MDS experience in a skilled nursing or long-term care setting preferred

• RAC-CT certification preferred or willingness to obtain certification as required

• Strong knowledge of MDS, RAI process, Medicare, Medicaid, PDPM, and regulatory requirements

• Excellent assessment, documentation, and analytical skills

• Strong organizational and time management abilities

• Excellent communication, leadership, and customer service skills

• Commitment to providing exceptional resident-centered care

Skills Required
