Dinwiddie Health & Rehab Center in Dinwiddie, VA is currently searching for an experienced, dynamic MDS Coordinator LPN to join our interdisciplinary team and MDS Department.

What’s My Primary Role?
- Provide leadership to interdisciplinary team for management of the MDS, Clinical Reimbursement, Care Plan processes and utilization management processes in accordance with current federal, state, and local requirements and regulations and company policies and procedures.
- Ensure that the MDS/RAI deadlines and accuracy requirements are met while ensuring that coding most appropriately captures each patient’s acuity and needs for accurate and optimal reimbursement.
- Assisting Center Leadership with Quality Improvement efforts, staff education, and regulatory compliance as it relates to the RAI process, Quality care, Reimbursement, and Quality Measures and Quality Reporting requirements.
- Demonstrate and promote a positive, collaborative, and caring team environment.
- Current LPN license with the Virginia Board of Nursing, in good standing.
- Two years’ experience with MDS process and/or current RAC-CT certification preferred.
- Achieve and maintain active RAC-CT status as condition of continued employment with time frame designated by CCR upon hire.
- Flexible, detail-oriented, ability to multi-task, meet deadlines and be able to make effective decisions and work as part of a service focus team.
- Ability to work flexible schedules.
PHYSICAL REQUIREMENTS:
- Prolonged periods sitting, standing, walking, working and frequently pulling, pushing, and bending.
- Must be able to lift 25 pounds at a time.
- Must be able to move 150 pounds with assistance.
Skills Required
- Current LPN license with the Virginia Board of Nursing in good standing
- Two years of experience with the MDS process
- Current RAC-CT certification
- Achieve and maintain active RAC-CT status as a condition of continued employment
- Flexible, detail-oriented, able to multitask, meet deadlines, make effective decisions, and work collaboratively
- Ability to work flexible schedules
- Ability to sit, stand, walk, pull, push, and bend for prolonged or frequent periods
- Ability to lift 25 pounds and move 150 pounds with assistance
