Salary: $87,000–$127,920 annually (based on licensure and experience)

We are currently seeking an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as an MDS Coordinator.
Position SummaryThe MDS Coordinator partners with the interdisciplinary care team to ensure accurate resident assessments, regulatory compliance, and optimal reimbursement. This role is responsible for coordinating the MDS process while supporting high-quality resident care through timely and accurate clinical documentation.
Key Responsibilities- Participate in the pre-admission process to obtain essential clinical information needed for accurate MDS optimization.
- Collaborate with the interdisciplinary team to review resident care and establish the most appropriate Assessment Reference Date (ARD) for Prospective Payment System (PPS) MDS assessments.
- Ensure resident assessments are completed accurately and on time in accordance with state and federal regulations.
- Complete precise MDS coding using information gathered through resident interviews, family discussions, staff collaboration, direct observation, and medical record review.
- Maintain and manage MDS assessment schedules to ensure regulatory compliance.
- Perform additional duties as assigned to support resident care and department operations.
- Current, active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Oregon.
- Previous MDS experience in a skilled nursing or long-term care setting preferred.
- Knowledge of the Resident Assessment Instrument (RAI) process and case management.
- Strong communication, organization, and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary team.
