Qualifications and Requirements:
- Responsible for the complete referral process, including identifying home health needs, verifying benefits, and coordinating care.
- Cultivate new and existing relationships resulting in company growth.
- Initiate and expand referral network.
- Comply with all aspects of the company's compliance and HIPPA regulations.
- One year of home health, marketing, or case management experience or a combination of each.
- Must possess a valid driver's license.

60,000-63,000 + Bonus
Skills Required
- At least one year of home health, marketing, or case management experience, or a combination of these
- Valid driver's license
- Ability to manage the complete referral process, verify benefits, and coordinate care
- Ability to cultivate relationships and expand referral networks
- Ability to comply with company compliance requirements and HIPAA regulations
