SHIFT AVAILABLE:
- Part-time Days: 6am-2:30pm, Mondays/Tuesdays and every other weekend
- Full-time Evenings: 2pm-10:30pm, includes every other Saturday and Sunday
- Part-time Nights 10pm-6:30am, includes every other Friday/Saturday

Your Responsibilities:
- Provide professional nursing services including resident assessments, vital signs, and medication administration
- Assist with restorative and rehabilitative care and daily living instruction
- Conduct regular rounds and communicate observations
- Document and report unusual or abnormal findings, care plan changes, and follow-up needs
- Supervise nursing personnel and provide job-specific guidance
- Communicate effectively with residents, families, and staff

- LPN from accredited or state approved school of nursing preferred.
- Valid Indiana LPN License
