As a Licensed Practical Nurse (LPN), you will provide direct patient care and support residents within our cottage-style units, ensuring high-quality, compassionate, and individualized care.
Key Responsibilities- Provide direct nursing care in accordance with care plans
- Administer medications and treatments as prescribed
- Monitor resident conditions and report changes to the clinical team
- Support and supervise CNAs to ensure quality care delivery
- Maintain accurate and timely clinical documentation
- Promote a safe, clean, and compassionate environment
- Collaborate with interdisciplinary team members
- Active, unencumbered LPN license in South Carolina (required)
- Current CPR certification required
- Must maintain all continuing education and remain in good standing with the State Board of Nursing
- Experience with PointClickCare (PCC) preferred
- Compassion for an underserved population
- Ability to be active and mobile throughout the shift (walking between cottages required)

Skills Required
