Provide and oversee nursing care for residents in a long-term/post-acute setting: assess, monitor, document conditions; administer medications; collaborate with interdisciplinary team; supervise STNAs and LPNs; respond to condition changes and advocate for resident needs.

Oversee daily operations of rehabilitation services for one or more facilities, supervise therapists and staff, ensure clinical documentation and regulatory compliance, implement and monitor treatment programs, manage productivity and billing, and support staff development and hiring.

Perform general administrative and clerical duties for a care facility including filing, meeting minutes, cash receipts, basic accounting support, HR/payroll assistance, supply management, incident reporting, and safeguarding resident protected health information.

Prepare and serve meals, set up and strip trays, operate and monitor dishwashing equipment, maintain kitchen cleanliness and food storage, prepare snacks, follow recipes and procedures, assist cook and dietary supervisor, and obtain/maintain food handler certification.

Coordinate and oversee resident care including plans of care, PT/OT, admissions, discharges, utilization review, and referrals. Communicate with residents, families, physicians, payers, and care teams. Maintain accurate documentation, ensure regulatory/licensing compliance, and manage daily priorities in a high-volume clinical environment.

Lead and manage the nursing department, ensure regulatory compliance and resident safety, recruit and train staff, coordinate clinical services (MDS, pharmacy), participate in QA and state surveys, and provide hands-on patient care as needed.

Provide regional clinical leadership across skilled nursing facilities, ensuring regulatory compliance, infection prevention, MDS oversight, survey readiness, and quality improvement. Support and train Directors of Nursing and MDS teams, lead audits and 5-Star initiatives, guide plans of correction, evaluate staffing and workflows, and serve as interim DON as needed. Frequent regional travel required.

Duties include scheduling, meeting minutes, cash receipts, filing, incident documentation, office supply management, assisting HR/payroll tasks, maintaining confidentiality of resident PHI, and supporting community relations and ad hoc administrative projects.

Prepare and serve meals, set up and strip trays, operate and monitor dishwasher, prepare snacks, clean and sanitize kitchen areas, store groceries, maintain floors and equipment, assist cooks and supervisor, and participate in staff training.

Evaluate and treat residents, develop and implement individualized occupational therapy plans, supervise COTAs and aides, train staff, document services, coordinate with care team and families, recommend durable medical equipment, and maintain compliance and safety.

Provide meal preparation, tray delivery and collection, maintain kitchen and dining cleanliness, follow safety and infection-control protocols, and support a positive dining experience for skilled nursing residents.

Provide occupational therapy treatments under an OT's direction to help residents improve ADLs, mobility, strength, and coordination. Monitor progress, document care, maintain a safe therapy environment, and collaborate with the interdisciplinary team in a skilled nursing/rehab setting.

Assist the supervising Occupational Therapist in delivering resident care, implement treatment plans, complete accurate documentation, maintain safety and infection control, educate residents and families, recommend durable medical equipment, train restorative aides, participate in care conferences, and contribute to quality improvement.

Provide direct, person-centered nursing care to residents and supervise nursing assistants. Perform administrative duties including admissions, transfers, discharges, documentation, daily census, incident reports, and charge slips. Ensure staff compliance with facility policies and regulatory standards, participate in quality assurance and facility surveys, coordinate care with other services, and lead unit/shift nursing personnel.

Assist with meal preparation, tray assembly, and service for residents in a long-term care setting. Maintain kitchen and dining cleanliness, follow food safety and sanitation regulations, support portioning and dietary guidelines, and work cooperatively with the care team to ensure nutritious, respectful meal service.

Provide resident-care under a supervising Physical Therapist, deliver treatments, document care, educate residents and families, assist with equipment ordering, support restorative training, and follow safety and regulatory requirements.

Prepare and serve meals, set up and strip trays, operate and monitor dishwashing equipment, clean kitchen surfaces and floors, store groceries, prepare snacks, record dishwasher chlorine and temperatures, and assist the cook and dietary supervisor as needed.

Provide direct personal care to residents including bathing, dressing, feeding, vital signs, mobility assistance, room upkeep, monitoring health changes, reporting to nurses, and following safety and HIPAA protocols.

Manage the nursing department, ensure resident safety and regulatory compliance, recruit/train staff, coordinate pharmacy and care planning, oversee budgets, participate in QA and state surveys, and provide hands-on patient care as needed.
