- Full-time, Monday-Friday, 8:00 AM - 4:30 PM
- Based with the Millersburg team, serving the southern portion of the region
- Extensive driving is required throughout the workday
- Participation in a weekend rotation (currently every 9th weekend, subject to change)
- Two holiday shifts per year

- Provide skilled nursing care under the direction and supervision of a Registered Nurse (RN) in accordance with the patient's plan of care and within the LPN scope of practice.
- Assist with nursing assessments by observing, collecting, and reporting patient information to the RN.
- Deliver care that addresses the physical, emotional, spiritual, and psychosocial needs of patients and their families.
- Monitor patient condition, recognize changes in status, and promptly communicate findings to the RN.
- Administer medications as prescribed using the correct dose, route, and frequency.
- Educate patients and caregivers on self-care techniques as directed by the RN.
- Document patient care, observations, and interventions accurately and promptly in the electronic health record.
- Collaborate with the interdisciplinary team to promote patient comfort, safety, and quality outcomes.
- Assist other departments and team members as needed to ensure comprehensive patient care.
- Participate in Continuous Quality Improvement initiatives.
- Serve as a preceptor for new team members when assigned.
- Current Ohio Licensed Practical Nurse (LPN) license in good standing
- One year of nursing experience preferred
- Current CPR certification
- Basic computer skills for electronic documentation
- Valid driver's license with a safe driving record
- State minimum automobile insurance coverage
- Ability to travel extensively throughout the service area, including daytime, evening, and inclement weather driving
