At St. Croix Hospice, the Licensed Practical Nurse provides direct patient care under the supervision of a Registered Nurse. This role includes following each patient’s individualized Plan of Care, delivering treatments, and collaborating with the care team to support hospice patients, provide comfort, and enhance overall quality of life.

QualificationsLPN Requirements & Core Responsibilities:
- Holds current Licensed Practical Nurse (LPN) license in state(s) of practice.
- Graduate of an accredited practical nurse or vocational nursing program required.
- Ability to pass background, consumer report and drug test.
- Provide direct patient care, follow the Plan of Care, and support end-of-life needs.
- Document all care accurately and promptly while educating patients and caregivers.
- Coordinate care by communicating patient needs to the interdisciplinary team.
- Manage pain and symptoms, use equipment safely, and maintain full HIPAA compliance.
- Must have reliable transportation, valid driver’s license, and proof of auto insurance

- Schedule: M-F 8:00am to 5:00pm, weekend visit coverage as needed, full-time position.
