Role and Responsibilities
The Client Relationship Specialist is a LPN who is responsible for developing and maintaining client relations and intake of direct patient referrals made to HCR.
Essential Functions
- Cultivate relationships with our physicians and hospitals to facilitate the best Home Care solutions for their patients.
- Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB).
- Monitor Intake Team email.
- Produce timely, accurate data entry.
- Responsible for entering orthopedic prescreens into HCHB according to agency protocol.
- Provide phone support to the department and triage of calls to Intake staff.
- Provide excellent customer service to internal and external customers.
- Participate in department and agency meetings.
- Other duties as assigned.

Education Requirements
- Graduate of an accredited practical nursing program and licensed to practice in NYS.
Qualifications and Requirements
- Minimum of 4 years of experience in a clinical setting.
- Exceptional customer services and communication skills required. Must be highly proficient and efficient working with a computer and be detail oriented and able to work in a fast-paced environment.
Work Environment
The Client Relationship Specialist is primarily in an office setting and may be exposed to outdoor conditions.
The working conditions are classified as sedentary work:
- Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
