We are looking for a Lead Case Manager for our Andover Day Program.
Responsibilities:
- Supervise and support Case Managers at a specific site including involvement in training oversight and performance development, corrective action on underperformance in conjunction with Site Directors. Conduct regular audits of case notes, service delivery, and compliance documentation to uphold quality standards and meet regulatory requirements.
- Attend and assist Case Manager in preparing for weekly Interdisciplinary Team (IDT) meetings to ensure person-centered planning, accurate documentation, goal development, and proper billing of day program services. Ensures MassHealth timelines are met on all service metrics.
- Review all completed MassHealth documentation, i.e. leveling tools, Prior Authorizations, Day Hab Service Plans and Service Needs Assessments. The Lead should review these documents for accuracy prior to uploading to the MassHealth platform.
- Assign cases based on workload and case acuity to individual Case Managers in coordination with Site Directors. Completes review and on-going support and evaluation of Case Manager performance.
- Additional responsibilities assigned as needed.
Education/Experience and Other Requirements:
- Bachelor’s degree preferred.
- A minimum of five years’ experience working in case management dealing with varied funding streams and regulatory requirements.
- Obtain and maintain certification in Medication Administration Program (MAP) Training, PABC and First Aid/CPR.
- Must have a valid driver’s license.
- Minimum of 1 year experience working with individuals with behavioral and/or clinical challenges.
- Schedule is typically Monday–Friday, 8:00 AM–4:30 PM; however, flexibility is required to work early mornings, evenings, or additional hours as needed.
- Work is performed in a fast-paced, high-stress environment with multiple responsibilities and short deadlines.
- Must be able to maintain composure and respond effectively in crisis situations.
