Northwestern Counseling & Support Services, Inc. (NCSS) is seeking a dedicated and compassionate individual to join our team as a JOBS Case Manager. Services provided in the Jump On Board for Success (JOBS) program model for youth and young adults with emotional and behavioral disabilities are meant to meet the goal of improving the employment and social functioning outcomes for those youth in transition. This position offers an opportunity to make a meaningful difference in the lives of individuals within our community.

Duties and Responsibilities:

  • Conduct interviews with clients to assess personal and family challenges, including but not limited to finances, employment, housing, and mental health issues.
  • Provide individual, family, or group counseling sessions to develop plans for meeting clients' needs and help them mobilize inner strengths and external resources.
  • Assist clients in modifying attitudes and behavior patterns by enhancing self-awareness and problem-solving skills.
  • Make referrals to community resources and other service organizations as needed to support clients' holistic well-being.
  • Develop individualized care plans, review service plans, and conduct follow-up to ensure the quality and effectiveness of services provided.
  • Deliver or assist in delivering Life Skills Instruction to individuals or groups as part of the support process.
  • Collaborate with community partners and attend meetings to support youth in accessing necessary resources and opportunities.
  • Provide support to help youth achieve their employment goals, including coordinating career exploration activities and hands-on learning experiences.
  • Engage with local businesses and organizations to create employment opportunities for youth, promote successful job placements, and accurately document services provided.

Hiring rate is based on experience, education, and internal equity. Final compensation will be determined in accordance with NCSS policy and applicable laws.

Qualifications

Bachelor's degree in social work, psychology, counseling, or a related field.

  • Previous experience working with youth in a social services or counseling capacity preferred.
  • Knowledge of community resources and services available to support youth preferred.
  • Strong communication, counseling, and interpersonal skills.
