Position Summary

The Independent Living Transition Specialist is responsible for providing core independent living skills for youth aged 16 through 22 that are in foster care or aging out of the foster care system, delivered through a trauma informed, wrap-around approach.  Key responsibilities include: ongoing assessments of life skills, transition planning with measurable benchmarks, monthly progress monitoring to support recommendations for movement within the Independent Living (IL) service continuum; access and utilization of internal and community resources when appropriate, intensive support for residents to become proficient in all aspects of IL programming. The position provides support in, identifying individualized goals to complete an Individualized Service Plan (ISP) for each resident to be reviewed quarterly, in the areas of: behavioral health & wellness, finances, nutrition, meal preparation, employment, education, vocation and support in securing permanent housing, Intensive supervision and guidance are critical adjunct skills to ensure all key components of independent living are achieved.


Some of the duties include:

  • Daily coaching and support to ensure goals are achieved
  • On-call support for residents and staff as necessary
  • Crisis Intervention support in collaboration w/Youth Services Residential Coordinator
  • Completion of IL assessments and other required documentation
  • Ensure residents are meeting program requirements i.e. scheduled classes, etc.
  • Participate in screening process and interviews to vet IL residents for programming
  • Maintain fluid & consistent communication between all parties involved
  • Ensure compliance with medication regimen if required
  • Monitor, record and administer resident medication as required
  • Facilitate the intake and discharge process, including ensuring all paperwork and documentation is complete, independent living clients as directed.

 

Qualifications

Our ideal candidate has a High School Diploma or GED; Associate Degree preferred, including a minimum of two years’ experience working with at-risk youth.  The candidate must also complete training requirements (CPR, First-Aid, Handle with Care, etc.), have a valid driver’s license and provide proof of insurance.
