The Director of Social Services works to provide overall direction to all aspects of the provision of patient social services including managing the members of the social work team. Establishes, implements, and evaluates goals and objectives for social services to meet and promote the standards of quality and contribute to the organizations mission and philosophy.

  • Directs all daily patient social service operations including providing direct oversight of the establishment and implementation of social service
  • Directs the implementation of improved work methods and procedures to ensure social services are delivered in accordance with local and state regulations
  • Ensures seamless home care service delivery by providing direct oversight of coordination of care with other clinical service
  • Assist with the leadership, supervision, and development of the clinical team, fostering a culture of accountability, compassion, collaboration, and excellence.
  • Assures proper documentation of social services in clinical records in compliance with state, federal, and CHAP
  • Collaborate with interdisciplinary team members and leadership to promote quality patient outcomes, operational excellence, and support the organization’s mission, vision, and values.
  • Maintains comprehensive working knowledge of the field of social work and shares information with appropriate organization
Qualifications
  • A Master's degree in Social Work. 
  • At least two (2) years management experience preferably in home care operations
  • At least 1 year of hospice experience
  • LCSW preferred but not needed
  • Demonstrates an ability to supervise and direct professional and administrative personnel
  • Knowledge of corporate business management, governmental regulations and CHAP standards
  • Demonstrates good communications and public relations skills
  • Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities
  • Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance

Skills Required

  • Master's degree in Social Work
  • At least two years of management experience, preferably in home care operations
  • At least one year of hospice experience
  • LCSW licensure
  • Ability to supervise and direct professional and administrative personnel
  • Knowledge of corporate business management, governmental regulations, and CHAP standards
  • Good communication and public relations skills
