Abound Health is seeking a Community Liaison & Business Development Manager to join our Growth team reporting to the Chief Growth Officer. In this role, you will expand and strengthen referral networks, generate new member leads, and support intake conversions across our IDD, Pediatric PDN, and Personal Care service lines. You will serve as the face of Abound within the community, driving growth through strategic outreach and relationship management. This role is remote but needs to travel 50% of time across Pennsylvania.

Key Responsibilities

  • Identify, cultivate, and maintain relationships with referral sources such as support coordination agencies, schools, and community partners.

  • Schedule and conduct in-person visits, presentations, and follow-ups with partner organizations.

  • Attend & host community events to represent Abound and promote brand awareness across IDD, Pediatric PDN, and Personal Care markets.

  • Generate new client referrals and warm leads; ensure prompt communication and handoff to intake and care coordination teams.

  • Track referral activity, conversion rates, outcomes, and trends using OnTarget, Microsoft Dynamics, and PowerBI.

  • Provide exceptional ongoing support to referral partners including performance updates, member outcome updates, and resolution of concerns.

  • Ensure full compliance with ethical referral practices, healthcare regulations, and internal policies.

Qualifications

  • Experience in business development, outreach, referral relations, or community engagement—preferably in healthcare, human services, or IDD/PDN programs.

  • Direct experience working in the PA IDD, Pediatric PDN, or Personal Care communities strongly preferred.

  • Strong relationship-building, communication, and presentation skills.

  • Ability to analyze referral data, track KPIs, and report on performance trends.

  • Familiarity with CRM or case management platforms (Microsoft Dynamics or similar).

  • Highly organized with the ability to manage multiple contacts, visits, and deadlines.

  • Valid driver’s license and willingness to travel locally for community visits and events.

Education

  • Bachelor’s degree in Business, Marketing, Human Services, Healthcare Administration, or related field preferred.

Compensation & Benefits

Base pay is determined by factors such as work location, experience, job-related skills, and relevant education or training. In addition to base pay, total compensation may include other forms of pay. We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits beginning on your 60th day of employment.
