Clinical Pharmacist

**Job Summary:**
The Clinical Pharmacist position involves integrating pharmacists as members of the care team within physician practices to manage patients' drug therapy effectively, ensure optimal care delivery, and improve health outcomes. This role supports the full physician panel, irrespective of health plan coverage.

**Responsibilities:**
- Provide comprehensive oversight of patient drug therapy.
- Execute clinical protocols permitted by state regulatory agencies.
- Optimize patient drug coverage benefits for appropriate care and cost reduction.
- Participate in clinic team meetings and treatment plan development.
- Offer patient and provider consultation in managing chronic disease states.
- Educate patients on appropriate medication use and disease self-management.
- Provide physician and staff education on clinical and formulary updates.
- Administer vaccines when necessary.
- Conduct post-discharge medication reconciliation for all transitions of care.
- Engage in Annual Wellness Visits for Medicare patients focusing on disease state progression and health management recommendations.
- Document activities within clinic electronic health record systems and other necessary clinical management systems.
- Collaborate with a multidisciplinary team by providing reports for care coordination.
- Contribute to Quality Improvement and Quality Assurance activities as required.
- Adhere to clinical policies, procedures, infection control principles, and confidentiality.
- Assist with administrative triage.
- Attend necessary meetings and undertake special assignments when requested by superiors.

**Minimum Qualifications:**
- Active and unrestricted pharmacist license in New York State.
- BS Pharmacy from an accredited college of pharmacy.
- 2+ years of experience as a practicing pharmacist.
- Certification in basic life support (BLS) and immunization administration.
- Excellent organization and communication skills.
- Ability to work collaboratively in a team environment.
- Strong attention to detail and ability to focus in a fast-paced setting.
- Proficiency in prioritizing tasks, problem-solving, and MS Office programs.
- Familiarity with electronic health record (EHR) systems.

**Preferred Qualifications:**
- Pharm.D. or other advanced pharmacy degree.
- Clinical residency in ambulatory care or similar field.
- 2+ years of experience in an ambulatory care/primary practice setting.
- Experience in managing drug therapy for patients with chronic conditions.
- Ability to identify and implement innovative solutions for improved efficiencies and outcomes.
- Bilingual proficiency.
- Experience with eClinical Works.

Salary Range: $107k - $115k

Note: this is an on-site position located in the Buffalo, NY area.
