Position: Clinical Performance Lead

We are seeking a highly motivated Clinical Performance Lead to support our clinicians and front-line staff in caring for our members affected by renal disease. As a Clinical Performance Nurse, you’ll play a vital role at Monogram Health. We are looking for a proactive individual that can help us continually improve and enhance our clinical performance.  

 

Responsibilities 
  • Patient Care Plan creation, modification, review, and approval in accordance with company policies. 

  • Routine auditing of clinician documentation and patient cases to ensure clinical quality and compliance with Monogram Health internal policies and applicable regulatory and contractual standards. 

  • Tracking and trending of team clinical performance to identify training and improvement opportunities under the direction of the Market Performance Manager. 

  • Training and communication to Care Managers and others related to new implementations or priority initiatives. 

  • Organization, facilitation, and follow-up for Case Rounds and High-Risk Review for the assigned market. 

  • Triage and manage clinical escalations from field clinicians and support staff to optimize patient safety and outcomes. 

  • Ensures timely and accurate communication with external patient care teams around priority patient needs, status changes and Monogram Health recommendations. 

  • Completion of priority patient outreach and/or visits to support market initiatives. 

 

Position Requirements  
  • Current unrestricted license as a Registered Nurse in assigned states/territories (compact licensing or multi-state licensing may be required) 

  • Basic Life Support (BLS) certification is required in this role. The company will support your certification completion through onboarding. 
  • At least 2-years of experience as a Registered Nurse, or equivalent clinical experience 

  • Comfort and proficiency using technology 

  • Bachelor’s degree preferred 

  • Experience working with patients with renal disease preferred
