The Clinical Nurse Program Supervisor (RN) is responsible for collecting, tracking, trending and monitoring data reviewing outcomes, compliance and identifying opportunities for improvement. Uses critical thinking skills to present findings and make recommendations to interdisciplinary health care team. Serves as clinical case manager and conducts new admission assessments as census dictates. Completes a clinical record review for current and new admissions.

Possess current CA license as registered nurse, BSN preferred.

Minimum of three years supervisory experience.

Experience leading within a Skilled Nursing facility

Demonstrated competence with use of electronic medical record, PCC preferred.

Ability to flex schedule based on facility needs.

Legally Authorized to work in the United States
