POSITION SUMMARY: Accompany persons served on medical appointments and advocate for the appropriate medical services for persons served. Provide direct care services to persons served in addition act as liaison between the residence and medical facilities, hospitals, laboratories and pharmacies. Ensure that the person’s served medical, dental or any other therapeutic appointments are carried out as required. Computer use/ability is required.
ESSENTIAL ACCOUNTABILITIES
1. Works in collaboration with the Residence Nurse and Residence Manager to ensure proper medical follow-up occurs for all persons served.
2. Accompanies persons served in obtaining medical and clinic care according to the person’s need. Advocates for persons served needs with physicians, lab technicians, psychiatrists, dentists and other providers of medical services. Ensures that clinical consults are completed appropriately prior to and following medical appointments.
3. Must be able to safely operate agency vans and/or vehicles as per the agency driving policy.
4. Communicates all immediate medical concerns to the Residence Manager and/or Residence Nurse.
5. Participates in weekly consult review meetings with the Residence Manager and Residence Nurse.
6. Ensures any and all medical documentation ordered and or medical interventions, i.e.: Blood work, EKG, X-ray reports etc., are obtained and placed in the persons binders for all medical consultations/ hospitalizations and or further placements.
7. Must be able to understand, articulate and document specific medical information.
8. Assists in training person served in activities of daily living and development of self-help and social skills. Provides training in ADL and other areas, performs these functions for people served requiring total support (diaper changing, ambulation, G-Tube, feeding, transferring from bed to chair using Hoyer lift), specifically staff will perform all aspects of the task i.e., showering, shaving, toileting, hand washing, mouth care; when applicable.
9. Implements goals designed to maximize each person’s level of independence, as determined by the IDT, including but not limited to: budgeting, grooming, nutritional planning, use of public transportation, use of community resources, shopping, and housekeeping. Required to display proficiency while entering person served data documentation in the EHR system (Electronic Health Record).
10. Account for and oversee all medication as per WellLife Network’s Medication Administration Policy. This includes the computerized EMAR (Electronic Medication Administration Record) system, where applicable. Required to display proficiency while entering person served data documentation in the EHR system (Electronic Health Record).
11. Communicates relevant information to other staff or supervisors; verbally and in writing, completing appropriate logs and incident reports.
12. Must be able to physically intervene with the person served when the person may cause physical injury to self or others; assist individual in responding to potentially dangerous situations; and administer emergency first aid.
13. Must successfully complete and perform all techniques required in AMAP, SCIP, CPR and First Aid.
14. Maintain confidentiality at all times.
15. Other duties as requested based on the needs of the facility and/or agency.

Qualifications

High School diploma or equivalent required; Associate’s degree in Human Services preferred. Experience working with the developmentally disabled and/or experience in the medical field. A valid NYS Drivers license is required.

Skills Required

  • High School diploma or equivalent
  • Associate's degree in Human Services
  • Experience working with the developmentally disabled and/or in the medical field
  • Valid NYS driver's license
  • Able to safely operate agency vans/vehicles per agency driving policy
  • Computer proficiency and ability to enter person data in EHR and EMAR systems
  • Ability to perform ADLs and total support tasks (diaper changing, ambulation, G-Tube care, feeding, Hoyer lift transfers, showering, toileting)
  • CPR and First Aid certification; successful completion of AMAP and SCIP training
