Provides peer-based mental health rehabilitation and recovery support to consumers. Teaches coping and self-help skills, advocates for recipients, develops support linkages, assists with treatment goals and community activities, conducts field visits, documents interactions and incidents, and collaborates with staff. The role requires empathy, lived experience with mental health or substance abuse, confidentiality, flexible scheduling, and travel between service locations.

Assesses program compliance with federal, state, and agency regulations through site visits, documentation reviews, audits, and incident investigations. Provides quality assurance training and technical assistance to program staff, communicates findings to leadership, evaluates training needs, and supports the well-being of individuals served. Requires travel across New York City and Long Island, periodic overnight and weekend on-call coverage, confidentiality, strong writing, and effective collaboration.

Provides psychosocial rehabilitation services to children and youth with behavioral health needs. Supports social, interpersonal, daily living, coping, and community integration skills through treatment-plan-based activities, home and community visits, groups, and collaboration with clinical providers. Documents services, monitors schedules and service units, reports safety concerns, attends training and supervision, and transports youth as needed.

Accompanies individuals to medical, dental, and therapeutic appointments; advocates with healthcare providers; coordinates follow-up and documentation; administers and tracks medications; provides direct support with activities of daily living, mobility, feeding, and personal care; enters records in EHR and EMAR systems; implements independence goals; completes incident reports; and responds to emergencies using approved intervention, CPR, and first-aid techniques.

Supports Assertive Community Treatment recipients in obtaining and maintaining Medicaid, Medicare, SNAP, SSI/SSDI, housing subsidies, and other benefits. Manages applications, recertifications, appeals, eligibility reviews, documentation, and coordination with clinical teams and community agencies. Provides case management, independent living skills support, crisis intervention, mental health support, transportation, and resource education. Maintains an assigned caseload, travels between program sites, documents services in Vellohealth, and participates in supervision and training.

Provide day-to-day support in a classroom setting for individuals with developmental disabilities: implement habilitation goals, assist with ADLs, collect/document progress, provide behavioral interventions using OPWDD techniques, supervise meals and activities, attend appointments, maintain a safe and stimulating environment, and complete required paperwork and certified safety techniques (AMAP, SCIP, CPR, First Aid).

Provide direct support and training to individuals with developmental disabilities in a classroom setting. Assist with activities of daily living, implement individualized goals, document progress, perform behavioral interventions using OPWDD techniques, support medical/psychiatric appointments, maintain safety and cleanliness, supervise meals, and complete required paperwork and electronic documentation.

Assists with classroom operations and supports people with developmental disabilities by implementing individualized goals, teaching daily living skills, providing behavioral interventions, documenting progress, supervising activities and meals, assisting with personal care, attending appointments, and maintaining a safe, clean environment. Requires physical interaction and emergency response capabilities, including CPR, First Aid, AMAP, and SCIP techniques.

Develop relationships with local employers to create job opportunities for individuals facing employment barriers. Provide career planning, job readiness training, resume and interview support, match participants to roles, negotiate accommodations, and deliver ongoing job coaching and retention services. Maintain participant records and ensure compliance with program and funding requirements.

Provide assessments, individual and group counseling, case management and linkage to services. Develop treatment plans with ACT team, facilitate groups, ensure documentation and OMH compliance, provide crisis intervention, coordinate entitlements and transportation, enter data into VelloHealth, and operate agency/own vehicle for home visits.

Provide IPS vocational services within an ACT program: conduct vocational assessments, rapid job search, employer engagement, job coaching, placement support, and documentation in EHRs while collaborating with the ACT team and community partners to support recovery and employment retention.

Provide direct care and medical advocacy for persons served: accompany to appointments, coordinate with nurses and providers, manage medications (EMAR), document in EHR, perform ADLs (including G-tube and Hoyer lift transfers), operate agency vehicles, complete required safety/CPR/First Aid trainings, and communicate medical needs and documentation to the care team.

Coordinate daily operations of the Bridger program, supervise staff, screen and assess clients for discharge, ensure compliance with agency and DOHMH/DMH regulations, manage schedules and documentation, oversee physical plant compliance, report incidents within 24 hours, remain on-call 24/7 for emergencies, and support program development and staff training.

Deliver IPS employment services within an ACT program: conduct vocational assessment, rapid job search, job development, employer engagement, and job coaching. Spend at least 65% of time in the community, enter data in NYESS/EHR/VelloHealth/CAIRS, document interactions within 48 hours, collaborate with ACT team, attend IPS trainings, and support client access to resources and job retention.

Provide IPS vocational services within an ACT program: rapid job search, employer engagement, job coaching, vocational assessments, job readiness training, placement coordination, and ongoing employment support. Spend majority of time in the community, document in EHRs (NYESS, CAIRS, VelloHealth), collaborate with ACT team and community partners, ensure regulatory compliance, and support participant recovery and retention.

Manage and coach a recruitment team, oversee sourcing, onboarding, compliance, and background clearance processes. Analyze recruitment data, maintain job descriptions and applicant records, support employer branding and outreach, and lead projects to improve recruitment operations and workforce planning across multiple locations.

Manage benefit application and appeals tracking, screen consumers for eligibility, assist with applications and appeals, liaise with government agencies, prepare reports and maintain consumer records, ensure HIPAA confidentiality, and provide training and coverage for department staffing needs.
