Supports classroom operations and individuals with developmental disabilities by implementing independence goals, providing daily living assistance, collecting performance data, applying approved behavioral interventions, supervising meals, attending appointments, completing documentation, and maintaining a safe, clean environment. Requires physical interaction with participants and successful completion of AMAP, SCIP, CPR, First Aid, and emergency response techniques.

Manages applications, recertifications, appeals, eligibility reviews, documentation, and coordination with clinical teams and community agencies. Provides case management, independent living skills support, crisis intervention, mental health support, transportation, and resource education. Maintains an assigned caseload, travels between program sites, documents services in Vellohealth, and participates in supervision and training.

Provide direct care and escort persons served to medical, dental, and therapeutic appointments; advocate with providers; document in EHR/EMAR; administer/oversee medications per policy; assist with ADLs including feeding, G-tube, transfers, and Hoyer lift; operate agency vehicles; complete required safety/first aid certifications; liaise with residence nursing/management and maintain confidentiality.

Provide assessments, individual and group counseling, case management and linkage to services. Develop treatment plans with ACT team, facilitate groups, ensure documentation and OMH compliance, provide crisis intervention, coordinate entitlements and transportation, enter data into VelloHealth, and operate agency/own vehicle for home visits.

Provide IPS vocational services within an ACT program: conduct vocational assessments, rapid job search, employer engagement, job coaching, placement support, and documentation in EHRs while collaborating with the ACT team and community partners to support recovery and employment retention.

Provide nursing assessment, medication administration, and ongoing psychiatric and medical coordination for behavioral health clients. Monitor labs and vitals, document care in the electronic health record, collaborate with prescribers and treatment teams, provide crisis intervention and supportive therapy, and ensure compliance with agency and regulatory requirements.

Provide direct care and medical advocacy for persons served: accompany to appointments, coordinate with nurses and providers, manage medications (EMAR), document in EHR, perform ADLs (including G-tube and Hoyer lift transfers), operate agency vehicles, complete required safety/CPR/First Aid trainings, and communicate medical needs and documentation to the care team.

Provide clinical supervision and direct care within interdisciplinary mental health and addiction clinics: manage clinic operations, carry caseload, perform intakes, lead team meetings, ensure regulatory compliance (OMH/OASAS), provide crisis intervention, coordinate care, and complete timely EHR documentation.

Coordinate daily operations of the Bridger program, supervise staff, screen and assess clients for discharge, ensure compliance with agency and DOHMH/DMH regulations, manage schedules and documentation, oversee physical plant compliance, report incidents within 24 hours, remain on-call 24/7 for emergencies, and support program development and staff training.

Deliver IPS employment services within an ACT program: conduct vocational assessment, rapid job search, job development, employer engagement, and job coaching. Spend at least 65% of time in the community, enter data in NYESS/EHR/VelloHealth/CAIRS, document interactions within 48 hours, collaborate with ACT team, attend IPS trainings, and support client access to resources and job retention.

Provide peer-based mental health support, teach coping and life skills, advocate for recipients, coordinate community supports, document interactions, and perform field-based visits.

Manage and coach a recruitment team, oversee sourcing, onboarding, compliance, and background clearance processes. Analyze recruitment data, maintain job descriptions and applicant records, support employer branding and outreach, and lead projects to improve recruitment operations and workforce planning across multiple locations.
