Overview

We are seeking a compassionate and dedicated HBCI Interventionist to be a part of this innovative program, working with children and their families. This position plays a crucial role in supporting youth with the dual diagnosis of intellectual and developmental disabilities who are experiencing mental health crises. You will provide short-term crisis intervention, therapeutic support, and collaborate with families and community members to help stabilize and improve the social, emotional, and behavioral functioning of the participants. You will have the opportunity to develop individualized treatment plans, provide therapy, and ensure that families feel supported throughout their child's journey toward healing and stabilization. This position requires flexibility, creativity, and a strong commitment to enhancing the well-being of individuals with complex needs.


This position is eligible for a sign on bonus of $5,000 and carries a maximum caseload of 3!

Position is 37.5 hours per week, hybrid is possible. Salary range is $50,000-$60,000 depending on education and experience.

Responsibilities
  • Design and implement a short term comprehensive crisis treatment response for each assigned participant and their family. 
  • Provide short term therapy to assigned participants . 
  • Act as a case manager to coordinate transitional connections that will enhance their social, emotional and behavioral functioning in school, at home and in the community and which reflects the perspectives of the student, family and other involved supports.  The program will reflect Racker’s values as defined in the Mission, Vision, Guiding Principles and Code of Conduct. 
  • Complete assessments that are recommended for improved diagnostic understanding, treatment planning and immediate crisis intervention 
  • Offer and deliver a full continuum of family support and therapy services and keep families fully informed on their child’s progress 
  • Gather and share treatment-related information with team members 
  • Maintain open and direct communication with team members and negotiate and resolve differences 
  • Engage and participate in clinical supervision 
  • Complete all documentation per Office of Mental Health and program guidelines 
  • Participate in an on call rotation and make contact with referred families within 24 hours of referral. 

Working Relationships:  Directly responsible to the HCBI Program Supervisor 

Qualifications
  • Qualifications: Master's degree in Social Work or related field. Will consider a Bachelor's degree with relevant experience.
  • Requirements: Use of a personal vehicle for flexible and timely response to meet with clients in the community. Use of a personal smart phone for communication and documentation.    
  • Skills/Abilities: Ability to provide effective mental health support to youth and their families and function as a member of a multidisciplinary team. 
  • Knowledge: Understanding of the needs of children with social, emotional and behavioral problems and the needs of their families.
