JOB DESCRIPTION:
SUMMARY OF POSITION
Provides care coordination activities to clients referred from the Alton CCBHC clinic. Responsible for completing engagement activities for clients and coordinating needed follow up activities within the organization or externally. Maintains up-to-date knowledge of the transition requirements for individuals who have experienced a recent hospitalization, ER visit, or transition in care. Assists clients with personal recovery goals, helps clients develop natural support systems, refers/links clients with resources, and coaches clients to advocate for themselves within their healthcare systems. Provides care as necessary to meet CCBHC core service obligations and in compliance with standards, regulations and laws, maintenance of appropriate documentation, etc. Works collaboratively with key leadership, including Statewide CCBHC Manager and Program Supervisors, to ensure services are provided with the highest quality to produce positive outcomes. Uses personal or loved one’s lived experience to provide support and share experience, strength, and hope appropriately.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Establish relationships and practice core coaching skills to transform client values/desires into action and maintain lasting change over time
- Share personal recovery experiences and develop authentic, and appropriate, peer-to-peer relationships while providing recovery training and outreach to individuals in services
- Learns about, can speak on, and coordinates with Centerstone programs and services and clients served.
- Coordinates with cross functional teams to ensure engagement and service activities are completed for clients within designated time frames.
- In conjunction with agency leadership, ensures collaboration across all programs and multiple levels of management to ensure clients receive proper referrals, both within Centerstone and with external partners.
- Work in consultation/collaboration with clients, family members and other providers to link clients with necessary resources or facilitate empowerment to access resources independently.
- Maintains appropriate communication with other employees and works as an effective, positive member of the organization.
- Represents Centerstone to internal and external stakeholders, funders, and community partners; creates relationships to successfully develop CCBHC clinics; provides community education and consultation as needed.
- Empower clients to self-manage chronic health conditions. Assist clients with finding solutions utilizing motivational interviewing. Assist clients in overcoming any barriers to goals, including access to care. Assist with navigating health care systems. Link clients to community-based social support/services and develop collaborative relationships with other departments, services, and community health care agencies. Help clients learn/navigate virtual platforms to assist in the completion of video telehealth visits. Assist service recipient in making discharge plans and provide follow up services as assigned.
- Utilizes evidence-based methodologies, as appropriate, that staff are adequately trained in the provision of such services, and that there is fidelity to the models to provide excellent clinical services
- Demonstrates an ability to manage crisis situations, including danger to self and others, to ensure the safety and stabilization of clients and to make appropriate referrals/linkages when clinically indicated. Does not leave crisis situation until client is in a safe environment or crisis is resolved.
KNOWLEDGE, SKILLS & ABILITIES
- Effectively communicate via written, verbal, in person and virtual methods.
- Skill in customer service techniques.
- Skill in responsible delegation, time management and prioritizing tasks.
- Skill in Microsoft Word, Excel, Powerpoint, and Outlook
- Strong interpersonal skills and ability to build positive relationships with colleagues and community partners
- Knowledge of role and function of various community resources
- Knowledge of confidentiality guidelines.
- Knowledge of internal and external referral procedures
QUALIFICATIONS
Education Level
- Associate’s Degree and 3 years lived experience required, HS Diploma/GED and five years of lived experience may be equivalent with Director approval
- Experience participating in their own personal recovery process in order to care for self. Must have the ability to share their recovery journey.
Years of Experience
- Must be at least 21 years of age
- Experience providing behavioral health services preferred
Certification/Licensure
- Must successfully complete all required background screenings. Ability and willingness to obtain credentialing if required.
