Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. 

We are seeking a Case Manager for one of The Doe Fund's permanent supportive housing for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. 

The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population.

Key Responsibilities:

  • Maintain a caseload and provide supportive case management services to these individuals
  • Maintain resident records in compliance with agency and funding standards 
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency 
  • Monitor and ensure residents  are linked to healthcare providers
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Facilitate groups and activities that promote health and independent living skills development
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health
  • Attend ongoing training to enhance the services provided to the residents
  • Assist with the completion of reports as needed
  • Other duties as assigned

The Ideal Candidate:

  • Bachelor’s Degree in Social Work or relevant field
  • Minimum of three years of post-bachelor experience
  • Ideal candidate must have a working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness
  • Excellent case management skills including knowledge of benefits in the NYC Metro area
  • Sensitivity to community issues and needs
  • Outstanding interpersonal and communication skills, both oral and written
  • Strong ethical standards, confidentiality, and personal integrity
  • Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)
