Provide intensive case management for formerly incarcerated and high-need clients: create and monitor Independent Living/Service Plans, coordinate referrals (mental health, substance recovery, benefits, primary care), document progress in databases, facilitate groups, perform crisis intervention, liaise with DHS and partners, assist management with reports and team support to meet program employment, housing, and retention goals.

Supervise client movement and meals, manage admissions supplies and OMNY card distribution, inspect and maintain facility and trainee rooms, coordinate rubbish/snow removal, train cleaning/laundry crews, document incidents, and request necessary supplies. Provide supportive services to residents of a transitional housing program.

Supervise up to three House Managers in a 200-bed residential facility, create schedules, manage shift coverage and overtime, enforce policies, document fire drills, manage bed tracking in DHS CARES, engage clients to attend services and work assignments, and maintain facility cleanliness and inspection readiness.

Manage a caseload of clients in the Ready, Willing & Able program, coordinate referrals to mental health, substance recovery, benefits and primary care, document progress in tracking databases, provide crisis intervention, facilitate groups, communicate with DHS and partner agencies, and support clients toward employment, housing, and program goals.

Provide administrative support for employee benefits and HR operations including enrollments, onboarding, filing, benefits communications, basic reporting, leave and workers' comp paperwork, first-level employee inquiries, and ad hoc HR projects under direction of Benefits Administrator and VP of People & Culture.

Facilitate workforce readiness classes and Career Club sessions, provide individualized job search coaching and graduate retention services, document client interactions in CTDB, coordinate with case managers and partners, and support employer engagement and reporting.

Provide supportive case management for chronically homeless clients in scatter-site housing: conduct mental health and psycho-social assessments, deliver counseling, crisis intervention, relapse prevention, facilitate support groups, develop service plans, document client contacts, coordinate life-skills training and community referrals to support housing retention.
