Program Overview:
To keep families together by providing safe, permanent housing, and ongoing accessibility to evidence-based trauma informed support services to Child Protection and Permanency families throughout our region.
Position Summary:
Provide support services and case management to families with a history of homelessness or housing instability. Assist parents to make a successful transition to self-sufficient living and improved individual and family stability. Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacy skills. Co-facilitate groups for parents, children and families. Link families to both formal and informal community resources. Work collaboratively with your KFT peers, DCP&P and community service providers. Complete documentation consistent with program and agency requirements. As an integrated health organization Acenda values the use of evidence-based practice or EBP's.
Key Responsibilities:
- Serve as the contact point, advocate and informational resource for clients, care team,
- family/caregivers and community resources regarding client's service plan
- Partner with clients to monitor adherence to their service plan, evaluate effectiveness,
- monitor progress in a timely manner and facilitate changes as required.
- Facilitate client linkages, referrals and access to appropriate treatment, community
- supports and medical or specialty providers according to the service plan
- Maintain an active caseload of approximately 10-12 families, providing in person in home therapeutic sessions to address treatment plan goals
- Educate patient and family/caregiver(s) about relevant community resources
- Cultivate and maintain relationships with external community providers and supports.
- Completes and submits accurate reports and chart documentation in a complete and
- timely manner according to program guidelines and established procedures.
- Ensures quality and accuracy of written and verbal communications.
- Support the Quality/Compliance/Risk function as required.
- Performs other related duties as assigned.
Requirements:
Bachelor's level practitioner with at least 2 years of experience in social service-related field preferred.
Must have and maintain a valid driver's license with an acceptable driving record
Must be able to exercise independent thinking and good judgment under all circumstances.
Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
- Base Pay: $19.23
- Sign-on Bonus: $500
- Schedule: Day shift
- Bilingual rate: An additional $1.50 added to the base pay if Spanish proficient.
