📝 What You’ll Be Doing

You will be responsible for the full lifecycle of case management, combining professional social work standards with community-level program organization.

Key Responsibilities:

  • Assessment & Evaluation: Conduct comprehensive in-home and in-office assessments of clients' social and emotional needs using standardized tools (e.g., PSA, EISEP-PATH).

  • Case Planning & Counseling: Develop and execute written case plans; provide individual/family counseling and guidance to preserve client autonomy.

  • Benefit Screening & Advocacy: Screen clients for entitlement eligibility, complete necessary applications, and fiercely advocate to secure benefits (Medicare, Medicaid, public assistance).

  • Service Coordination: Arrange critical direct services including homecare, transportation, meals on wheels, legal aid, and emergency psychiatric or medical care.

  • Financial Protection: Evaluate clients' capacity to manage their own affairs; protect seniors from financial exploitation and assist with guardianship or emergency relief fund disbursements.

  • Program & Event Organization: Organize daily community activities and monthly events; facilitate group counseling, community education, and caregiver support groups.

  • Staff Supervision: Supervise community aides, Title V workers, homemakers, home health aides, interns, and volunteers.

  • Administration & Advocacy: Maintain precise electronic case records, reports, and statistics; represent and interpret JASA’s mission at community conferences and meetings.

  • Education: Graduate of an accredited college or university with a BSW, BA, or BS degree (or equivalent).

  • Technical Skills: Computer literacy with the ability to manage electronic records; strong writing and data organization skills.

  • Communication: Proficiency in English required; knowledge of Yiddish, Russian, Spanish, or Patois is highly helpful.

  • Travel: Ability to travel to clients’ homes using mass transit and navigate physical barriers (stairs, etc.) in various residences.
