Community Health Alliance is seeking compassionate case managers. As a Case Manager, you will provide direct support to individuals living with mental health conditions by coordinating care, teaching daily living skills and fostering recovery through individualized services. This role is community based and includes home visits, service coordination and collaboration with providers, families and other supports.

What You'll Do
As a Case Manager, you will:
- Coordinates assessments, treatment planning or crisis intervention services.
- Facilitates healthcare, behavioral health and other services and supports as part of care coordination.
- Provides training and facilitates linkages for the use of community resources.
- Monitors symptoms and service delivery to ensure client needs are met.
- Obtains or assist clients in obtaining necessary services for basic necessities and services.
- Assists clients in accessing self-help support groups and increasing social support networks.
- Provides appropriate interventions to aid clients in the restoration of daily living skills.
- Coordinates and assist in crisis management and stabilization.
- Collaborates and communicates effectively with treatment team for the coordination of care.
Required Qualifications
- High School Diploma and 3 years of relevant work experience in behavioral health or Bachelor’s degree in a social sciences field or related field.
- An understanding of mental illness, psychiatric symptoms, and impact on functioning and behavior.
- An understanding of how to therapeutically engage a person with mental illness.
- Valid driver's license, car insurance, and insurable driving record required.
- Willingness to travel throughout the assigned service area.
- Must have reliable transportation.
- An understanding of the community mental health system.
