SUMMARY 

The Case Manager works with the care team to appropriately identify care needs and prepare client for discharge planning. Will work with client employers and insurance payors to coordinate necessary leave of absence documents. The overall goal is to improve outcomes, increase client satisfaction, cost-effectiveness, and promote an informative approach with the client to decrease their concerns around their care. 

JOB FUNCTIONS 

  • Collaborates with the Financial Department to ensure all required insurance payments are collected and disseminates pertinent client information that would prevent payment collections. 
  • Participates in assigned trainings and proactively seeks knowledge to build competency. 
  • Maintains knowledge of current regulatory and accreditation standards, along with federal, state, and local codes pertinent to this role and addiction treatment.  
  • Case management duties should include court/legal issues, FMLA/STD, appointments, aftercare, communal services, and speaking with client employers and insurance payers to coordinate necessary documents. 
  • Conducts weekly individual case management/counseling session and provides weekly family check ins.  
  • Ensures all regulatory standards, requirements, and guidelines, including licensing boards, accreditation authorities, and federal, state, and local regulations are adhered to. 
  • Adheres to scheduled work hours with regular and predictable attendance. 
  • Collaborates with care team in the development and implementation of clients’ treatment and intervention programs that they can understand and comply with. 
  • Participates in crisis intervention and conflict resolution actions, including suicide assessments, partner abuse, child and elder abuse screening; takes appropriate measures to resolve crisis situations; implements mandatory reporting procedures in accordance with regulations (including HIPAA) and policies. 
  • Monitors clients’ conduct throughout the day. 
  • Ensures clients are attending appropriate sessions and are on time. 
  • Collaborates with clinical team and attends and participates in clinical team meetings. 
  • Conveys clients’ needs to medical, operational, and other clinical staff.  
  • Maintains a high level of flexibility with the daily flow of work tasks. 
  • Maintains proper chain of command. 
  • Assists in the facilitation of group sessions as assigned.  
  • Collaborates with other staff members to plan and facilitates client activities.  
  • Assists in client assessment and clinical decision regarding treatment planning. 
  • Meets with clients as needed to monitor progress and treatment planning. 
  • Assists in facilitating discharge and aftercare planning. 
  • Facilitates family sessions as assigned. 
  • Maintains appropriate documentation as required per Company’s Policies and Procedures. 
  • Maintain proper chain of command. 
  • Attend and participate in all staff meetings/in service training.  
  • Maintain a high level of flexibility with the daily flow of work tasks. 
  • Complete all other duties as directed by leadership. 
Qualifications
  • Minimum of Master’s Degree in Psychology, Mental Health Counseling, Social Work or related field REQUIRED.
  • Two years (2) of working experience preferred.
  • Chemical Dependency experience strongly preferred.
  • Most possess one of the following Licenses: Certified Alcohol and Drug Counselor (CADC), Licensed Clinical Alcohol and Drug Counselor (LCADC), Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW),

Skills Required

  • Master's Degree in Psychology, Mental Health Counseling, Social Work or related field
  • Two years of working experience
  • Chemical dependency experience
  • Possess one of: Certified Alcohol and Drug Counselor (CADC), Licensed Clinical Alcohol and Drug Counselor (LCADC), Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW)
