We're looking for a Case Manager to join our team of difference-makers. This isn't a desk job where every day looks the same.   • Becoming a trusted ally for youth and families.
  • Helping clients identify goals and create a roadmap to achieve them.
  • Connecting people with resources they didn't know existed.
  • Partnering with schools, providers, community agencies, and support systems.
  • Turning "I don't know what to do" into "I've got a plan."
  • Supporting clients through challenges, setbacks, and victories.
  • Helping youth build skills for independence, education, employment, and everyday success.
  • Coordinating care so everyone stays focused on what matters most—the client.
  • Managing crises with calm, confidence, and compassion.
  • Creating service plans, documenting progress, and keeping everything moving forward.
  • Celebrating wins—big and small.

Qualifications🎓 What You'll Need
  • Bachelor's degree in Human Services or related field OR
  • High School Diploma/Associate Degree plus 3 years of relevant experience
  • QMHS certification
  • Reliable transportation and willingness to travel throughout the region
