Case Manager III
 

We are seeking a Case Manager III to work on our Dual Case Management (Dakota County) team to provide the highest level of case management and care coordination through plan development, monitoring, evaluation, and advocacy. Principle areas of responsibility include acting as lead for the program, provide training to other case management staff, and maintain referral source relationships. This position will also support other members of the team in achieving these objectives.


Hours: Full-Time, Monday- Friday, 8am-4pm


Locations: Dakota County, Minnesota | Hybrid **Physical presence is required**


Hiring Ranges: $28.00-$30.00/ Hourly, based on qualifications
 

Hiring Bonus: $1,000 to external candidates

Main Job Duties:

  • Physical presence in the Dakota County is required.
  • 10-12 case load
  • Manage and/or provide consultation on the most complex cases ensuring consistent practices and outcomes.
  • Partner with the program leadership on cultivating and maintain relationships with potential and current referral sources and community partners.
  • Provide advanced knowledge and guidance on assisting clients in the multiple phases of applying for and receiving services and benefits.
  • Assist program management on training case managers on policies, procedures and best practices, taking an active role in onboarding new case managers.
  • Perform assessments that lead to individual plan development that result in the client receiving access to necessary mental health and related services.
  • Facilitate the development and implementation of a goal plan that addresses the needs of the clients, and is based on their development/goal plan and is culturally appropriate.
  • Provide referrals to resources necessary to implement the agreed upon goal plan. This may include outreach and consultation with other partners and providers.
Qualifications

  • Passing all background and requirements for facility and position; and
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance.

Education and Experience:

  • Masters Degree, or
  • Bachelors Degree in the behavioral sciences or related field (i.e. social work, psychology, or nursing from an accredited college or university), and 4,000 hours of supervised experience in the delivery of services to people with serious and persistent mental illness.
  • Minimum of two years prior work experience as a case manager or other direct service role in the human services field working with:
    • Advanced cases (examples may include but are not limited to: working with clients on high commitments, high needs, significant county involvement, high profile and/or clients needing unique solutions); or
    • Combination of advanced cases and dual case management
  • In lieu of 2 years advanced or combined advanced/dual case management experience, 1 year of dual case management.
  • In addition, employees entering and holding this position must be able to demonstrate mastery in the skills and required competencies.

Preferred Experience:

  • Masters Degree, or
  • Bachelors Degree in the behavioral sciences or related field (i.e. social work, psychology, or nursing from an accredited college or university), and 4,000 hours of supervised experience in the delivery of services to people with serious and persistent mental illness.
  • Minimum of two years prior work experience as a case manager or other direct service role in the human services field working with:
    • Advanced cases (examples may include but are not limited to: working with clients on high commitments, high needs, significant county involvement, high profile and/or clients needing unique solutions); or
    • Combination of advanced cases and dual case management
  • Waiver and targeted case management experience with adults with severe persistent mental illness.

Skills Required

  • Physical presence in Dakota County, Minnesota
  • Pass all background checks and facility/position requirements
  • Pass Federal OIG screening (not excluded from federally funded health programs)
  • Valid and current driver's license and auto insurance
  • Master's degree OR Bachelor's degree in behavioral sciences (social work, psychology, nursing) plus 4,000 hours supervised experience with serious and persistent mental illness
  • Minimum two years prior work experience as a case manager or direct service role working with advanced cases or combination of advanced cases and dual case management
  • Alternatively, one year of dual case management experience may substitute for the two-year advanced/combined requirement
  • Demonstrated mastery in required skills and competencies for the position
  • Waiver and targeted case management experience with adults with severe persistent mental illness
