Provide crisis intervention, coordinate services and benefits, assist with admissions and treatment/discharge planning, facilitate skill-building and appointments, maintain schedules and documentation, and support client recovery through person-centered practices.

Supervise a team of six Waiver Case Managers delivering Medicaid-funded, person-centered services. Responsibilities include staff supervision, hiring, training, performance feedback, caseload oversight, direct client services as needed, stakeholder coordination, and maintaining program quality and compliance with county and state standards. Hybrid role with regular in-person presence in the Twin Cities area.

Provide targeted case management for adults with chronic mental illness by performing functional assessments, developing and reviewing person-centered plans, coordinating referrals and continuity of care, documenting interventions and records, and attending client meetings to monitor services.

Provide Waiver case management for adults with serious mental illness: develop person-centered support plans, authorize and coordinate services, attend bi-annual client meetings, maintain records, make referrals, and collaborate with partners to ensure continuity of care.

Provide targeted case management for adults with chronic mental illness in the Deaf and Hard of Hearing community. Perform functional assessments, develop and monitor individual plans, coordinate referrals and continuity of care, document interventions, attend client meetings, and work autonomously in the community. Must maintain records for funding and ensure service effectiveness.
