Provide front-desk and administrative support at an outpatient mental health clinic: greet clients, manage calls, schedule appointments, collect co-pays, enter insurance and demographic data into the EHR, and support clinic projects and reporting.

Provide person-centered crisis coordination and support for clients in Crisis/IRTS: engage clients, perform crisis intervention and de-escalation, coordinate services and benefits, assist with appointments and telehealth, complete documentation, and develop individualized treatment and discharge plans to support client recovery and program safety.

Supervise and develop staff in a 24-hour hybrid crisis/IRTS mental health program, ensure regulatory and billing compliance, support program quality and budget targets, and provide direct client care including assessments, treatment planning, and therapeutic engagement during the evening shift.

Supervise six CADI waiver case managers, providing coaching, hiring, training, schedule coordination, performance feedback, quality assurance, stakeholder collaboration, and occasional direct client services to ensure person-centered Medicaid-funded supports.

Coordinate crisis and IRTS services, engage clients using person-centered recovery principles, provide crisis intervention and de-escalation, assist with benefits and community resource navigation, facilitate appointments and telehealth, conduct orientation and admissions, develop individualized treatment and discharge plans, maintain documentation, and manage daily program schedules and staffing coverage.

Provide targeted case management for adults with severe or persistent mental illness: assess needs, develop person-centered support plans, coordinate and authorize services, maintain documentation, build collaborative relationships with clients and community partners, and attend periodic client meetings. Work autonomously with onsite presence in Dakota County.

Provide waiver case management for adults with chronic mental illness: assess needs, develop and review individualized plans, make referrals, maintain records, document interventions, and attend client meetings to support independent living.

Provide case management for children (ages 5-17) with serious mental health diagnoses: assess needs, develop and monitor plans, link to services (therapy, medical, educational, insurance), and maintain regular client contact and coordination with providers and families.

Provide targeted case management for adults with serious mental illness: develop person-centered plans, coordinate services and referrals, document interventions, maintain community partner relationships, and conduct outreach and monitoring to support independent living.

Provide waiver case management for adults with chronic mental illness: conduct functional assessments, develop and review individual plans, make referrals, maintain documentation, attend client meetings, and ensure continuity of care while working autonomously in the community.

Provide advanced case management and care coordination for complex mental health clients: lead program activities, train and support staff, develop and monitor individualized plans, maintain referral relationships, and facilitate access to services and benefits.

Provide targeted case management for children (5-17) with serious mental health diagnoses: assess needs, develop and monitor individualized plans, link and refer to clinical and community resources, monitor service effectiveness, maintain regular client contact, provide temporary coverage during transitions, and serve as an internal resource and subject-matter expert for other case managers.

Provide waiver case management for adults with chronic mental illness or related conditions: develop person-centered support plans, authorize services, monitor care with bi-annual visits, maintain partner contact and documentation, and coordinate referrals to promote independent community living.
