Case Manager II


People Incorporated is seeking a Case Manager II to join our Waiver Case Management Anoka County team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. We value work-life balance and have manageable caseloads that are around 30 clients on average!


Hours: Full-Time, Monday- Friday, business hours 8am-4:30pm


Locations: Anoka County | Minnesota | Hybrid


Hiring Range: $24.20 - $25.75/ Hourly based on qualifications


Hiring Bonus: $1,000 to external candidates

Main Job Duties:

  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
Qualifications

Requirements for All Case Manager II positions:

  • Valid driver’s license, excellent driving record and current auto insurance

Education and Experience Required for Anoka County Waiver Case Manager II positions:

  • Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field;

OR

  • Bachelor’s Degree from an accredited four-year college with a major in any field, AND
  • One year of experience as a social worker/case manager/care coordinator in a public or private social service agency

Preferred Education and Experience for Anoka County Waiver Case Manager II positions:

  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND
  • One year of experience in waiver case management or care coordination with adults experience severe mental illness.

Skills Required
