Position Summary
The Case Manager provides family-centered case management and supportive services to families experiencing homelessness or housing instability who are enrolled in the I'Tollai "The Bridge" Program (ITP). The Case Manager works collaboratively with families to assess needs, develop service plans, coordinate resources, address barriers to housing stability, and support families in achieving safe, stable housing and improved well-being. This position works closely with program staff, community partners, schools, shelters, landlords, and service providers to promote positive outcomes for children and families.
Essential Job Functions
• Maintain an active caseload of families enrolled in the ITP program.
• Conduct screenings, intake, assessments, and reassessments to determine participant needs and eligibility.
• Develop, implement, and monitor individualized service and housing stabilization plans.
• Provide ongoing case management using Housing First, trauma-informed, strength-based, and family-centered approaches.
• Assist families in obtaining and maintaining safe, stable housing.
• Coordinate referrals and access to housing, public benefits, healthcare, behavioral health, employment, education, childcare, and other supportive services.
• Conduct home visits, shelter visits, and community-based services as needed.
• Collaborate with the Housing Navigator and community partners to support housing placement and retention.
• Maintain accurate, timely, complete, and confidential participant records and documentation in accordance with program, agency, and funder requirements.
• Monitor participant progress and adjust service plans as needed.
• Participate in case conferences, staff meetings, training, and quality improvement activities.
• Participate in outreach and community engagement activities to identify and connect eligible families with services.
• Maintain knowledge of community resources and services available to families experiencing homelessness.
QualificationsEssential Qualifications:
Education
• High School Diploma or equivalent required.
• Associate or bachelor's degree in social work, human services, psychology, sociology, counseling, criminal justice, or a related field preferred.
• Equivalent combinations of education and experience may be considered.
Experience and Competencies
• Experience providing case management, housing services, homeless services, family services, or related human services preferred.
• Knowledge of Housing First, trauma-informed care, and strength-based service approaches preferred.
• Experience working with families and children preferred.
• Strong communication, organizational, documentation, and problem-solving skills.
• Ability to manage multiple priorities and maintain confidentiality.
• Experience with electronic case management, client information, or data tracking systems preferred; proficiency with Microsoft Office required.
• Valid driver's license required. Ability to travel locally to participant homes, partner agencies, shelters, and other community locations.
• CPR and First Aid Certification preferred, or ability to obtain certification after hire.
