JOB DESCRIPTION:
SUMMARY OF POSITION
Join an innovative role co-located within the Hickman County Health Department, expanding behavioral health access for patients receiving medical care. Coordinate services, provide coaching, and connect individuals with resources to live healthier, fuller lives. Teach the link between mental and physical health and become a trusted guide in your community.

ESSENTIAL DUTIES & RESPONSIBILITIES
- Help patients navigate the health care system through coaching, education and support
- Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness
- Refers, advocates, and links client and family for additional services as identified.
- Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills
- Provides supportive and educational case management services to develop daily living and coping skills.
- Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.
- Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
- Connect with clients face to face in the office and community, telehealth, and by phone to address needs
- Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.
- Attend weekly staffing and team meetings and completes assessments
- Assist clients with developing natural support systems.
- Completes all required agency trainings to maintain 100% compliance.
- Maintains departmental productivity expectations.
- Attend and participate in clinical supervision.
KNOWLEDGE, SKILLS & ABILITIES
QUALIFICATIONS
Education Level
Bachelor’s degree in psychology, social work or related field required
Years of Experience
Experience with Electronic Health Record preferred
Certification/Licensure
Valid driver’s license in good standing required
