In this role, you will coordinate care and services for individuals navigating mental health challenges, providing coaching, education, and support to improve daily functioning and overall wellbeing. You will connect clients with resources, advocate for their needs, and help them build skills that support long-term recovery—focusing on the vital connection between physical and mental health.

Key Responsibilities:
• Build meaningful relationships with clients through face-to-face, telehealth, and phone interactions to support mental health recovery and wellness
• Empower clients by teaching coping skills, life management strategies, and the importance of whole-person care
• Coordinate behavioral health and primary care services to remove barriers and improve outcomes
• Connect clients and families to community resources, advocating for access to needed services
• Utilize motivational interviewing, behavioral management techniques, and crisis intervention to support client progress
• Develop, implement, and update individualized treatment plans in collaboration with clients and families
• Document services accurately and timely within the electronic medical record (EHR) system
• Participate in team meetings, clinical supervision, and ongoing assessments to ensure high-quality care
• Support clients in building natural support systems within their community
• Maintain productivity standards and complete required training to ensure compliance

Education, Experience, and/or Credential Qualifications:
• Bachelor’s degree in Psychology, Social Work, Counseling, or a related human services field required
• Experience using Electronic Health Records (EHR) systems preferred
• Knowledge of case management, care coordination, and mental health services preferred

Additional Qualifications:
• Valid driver’s license in good standing required
• Strong communication, active listening, and interpersonal skills
• Ability to work independently and collaboratively within a team environment
• Passion for behavioral health, patient advocacy, and community support services
• Flexibility to work in clinic, community, and home-based settings

Physical Requirements
• Sit for extended periods (approximately 90% of the time)
• Stand occasionally (approximately 10% of the time)
• Perform occasional bending, kneeling, and squatting
• Drive occasionally for client visits and community-based work
• Lift and carry items occasionally as needed

Pay Range:

$19.10--$26.80
