POSITION SUMMARY:

The Case Manager is responsible for providing intensive, person-centered case management services to veterans and service members who are identified as at risk for suicide. This position assesses participant needs, reduces barriers to care, coordinates services, and supports stabilization, recovery, and long-term wellness. The Case Manager works closely with program, intervention, and peer support staff to ensure participants receive timely access to mental health care, substance use treatment, housing, employment support, benefits, and other wraparound services. Essential functions include those listed below.

ESSENTIAL JOB FUNCTIONS:

  • Conduct comprehensive, person-centered assessments to identify participant strengths, needs, risks, barriers, and service priorities.
  • Develop, implement, monitor, and update individualized service plans in collaboration with participants and the interdisciplinary program team.
  • Provide ongoing case management, care coordination, advocacy, follow-up, and resource navigation for veterans and service members at risk for suicide.
  • Connect participants to behavioral health, substance use treatment, medical care, housing, employment, benefits, transportation, legal, and other community-based resources as appropriate.
  • Maintain regular participant contact and promptly address barriers that may interfere with engagement, safety, stabilization, or continuity of care.
  • Coordinate with peer support staff, health care providers, community partners, veteran-serving organizations, and other referral sources to promote continuity of services.

 

 

 

  • Support suicide prevention and risk-reduction activities within the scope of the psooition and follow established program protocols when safety concerns are identified.
  • Provide trauma-informed, culturally responsive, strengths-based, and participant-centered services while maintaining appropriate professional boundaries.
  • Document assessments, service plans, contact, referrals, progress, outcomes, and other required information accurately and within established timeframes.
  • Participate in case conferences, staff meetings, training, quality improvement activities, and grant-related data collection and reporting  as assigned
  • Maintain confidentiality and comply with organizational policies, funder requirements, and applicable laws and regulations.
  • Conduct community-based outreach and participant visits as needed.
  • Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
  • Perform all other duties as assigned.
Qualifications

ESSENTIAL QUALIFICATIONS:

Experience and Competencies:

  • Strong interpersonal, engagement, advocacy, and relationship-building skills.
  • Ability to organize and prioritize a caseload, coordinate multiple services, and complete documentation timely.
  • Ability to communicate effectively with participants, families, providers, community partners, and multidisciplinary teams.
  • Sound judgment, problem-solving ability, and the ability to respond appropriately to urgent participant needs.
  • Basic computer proficiency and ability to use electronic records, databases, email, and standard office applications.
  • Excellent communication skills;
  • Ability to exercise good judgment and discretion;
  • Ability to work well in a team environment;
  • Military/Veteran status required.
  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality;
  • Professional appearance and demeanor;

 

 

 

 

 

  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required; and
  • Ability to obtain and maintain a criminal history background check.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

EDUCATION and/or EXPERIENCE:

  • Minimum of two (2) years of experience in case management, behavioral health, veteran services, or a closely related area.
  • Experience working with high-risk populations, including individuals experiencing mental health and/or substance use disorders.
  • Knowledge of suicide prevention principles, trauma-informed care, community resources, and challenges affecting veterans and service members.

LICENSURE/CERTIFICATION:

  • Not applicable for this position.

WORKING CONDITIONS:

  • Work is primarily performed in an office setting.
  • Regular travel throughout the assigned service area is required to support outreach, community partnerships, participant services, meetings, and program activities.
  • Travel between WestCare facilities, VA facilities, community partner locations, and outreach sites may be required.
  • Position requires regular interaction with Veterans and individuals who may be experiencing behavioral health concerns, emotional distress, significant psychosocial stressors, or suicide risk.
  • Some evening or weekend work may be required to support outreach events, community activities, or program needs.
